just to hazard a flippant comment in this direction -- institutions/systems are abstractions in service of people organizing their actions together, and where these abstractions break down are at the boundaries.

The usual rules don't apply, and the pun is intended, because it is yet a strong, provisional, situated, and unsupported claim.
