The qualifiers Judy Dianondstone has used here on assessment articles
suggest a not very well hidden agenda. Nevertheless here are some
suggestions:
The Whole Language Evaluation Book, Heinemann, Goodman, Goodman and Hood
is full of articles by teachers on assessment.

The Assessment Supplement of The Whole Language Catalogue (McGraw Hill-
Macmillan,SRA div) by Goodman, Bird, and Goodman has a wide variety of
assessment procedures used by teachers and others. There is also a forms
book of reproducible forms used by teachers that accompanies the
supplement.

You might also like to look at the Primary Language Record published by
the Centre for Prinary Language Education in London, Myra Barrs is the
editor.
